Our Approach

Professional Referral & Care Coordination

We maintain active collaboration with hospitals and discharge planners, Community Services Boards (CSBs), case managers, social services agencies, and legal or court systems to support coordinated care.

When a participant is referred, our team follows a structured admission process that begins with referral submission and assessment. Based on clinical evaluation, a treatment plan is developed, and progress is monitored throughout the program to ensure accountability and continuity of care.
